- Patients and caregivers are told to talk with the doctor and staff, such as a discharge planner, social worker, or nurse, about each item.
- Items include asking where care will be provided after discharge (for example home health care) and whether medical equipment is needed and who will arrange it.
- Patients are told to write down every drug they take and review the list with staff before leaving.
