Money Follows the Person

Money Follows the Person (Take Me Home) provides transition services including home modifications, furnishings, moving expenses, and past-due utility assistance for West Virginia residents transitioning from nursing facilities and hospitals to community homes.

Eligibility

• Care recipient must have a qualifying disability
