Program of All-inclusive Care for the Elderly (PACE)

Program of All-inclusive Care for the Elderly (PACE) provides comprehensive services to individuals 55 years of age or older who require nursing home level of care but can live safely in the community. Services are delivered in community-based centers and participants' homes.

Eligibility

• Care recipient must have a qualifying disability
• Care recipient age: at least 55

How to apply

• Call the intake representative to be screened over the phone for eligibility; a tour of the center or a home visit is offered, and if the applicant wishes to enroll the intake representative assists with paperwork.
