Money Follows the Person

Money Follows the Person provides one-time financial assistance up to $9,000 to eligible individuals transitioning from qualified inpatient facilities (hospital or nursing home) to community housing. Eligible applicants must be Vermont residents 18 years of age or older who have been in a facility for 60 or more days.
