Choices for Care

Choices for Care provides Medicaid-funded long-term services and supports, including home and community-based services, for Vermont residents age 65 and older, individuals age 18 and older with physical disabilities, and Medicaid-eligible children and adults.

Eligibility

  • U.S. citizenship or qualified immigrant status required
  • Care recipient must have a qualifying disability
  • Care recipient age: at least 18

How to apply

  • Complete the 202LTC application form and mail it to Green Mountain Care Application and Document Processing Center, 280 State Drive, Waterbury, VT 05671-1500
