Vermont Choices for Care Program

Vermont's Choices for Care Program provides long-term services and supports to Vermonters age 18 and over who need nursing home level of care. Eligible people may receive services at home, in a family's home, an Adult Family Care home, Enhanced Residential Care, or a nursing facility, subject to clinical and financial eligibility.

Eligible people may choose services in their home, a family's home, an Adult Family Care home, Enhanced Residential Care, or a nursing facility. Applicants must meet both clinical and financial eligibility for long-term care Medicaid.
