1915(i) State plan HCBS

1915(i) State plan HCBS provides Transition Assistance Services and Psychosocial Rehabilitation Services to adults over 18 with Serious Mental Illness whose income does not exceed 150 percent of the Federal Poverty Level.

Eligibility

  • Household income at or below 150% of the Federal Poverty Level
  • Care recipient must have a qualifying disability

How to apply

  • An individual can be referred to the HCBS-AMH program if they have a primary diagnosis of SMI
