Money Follows the Person

Money Follows the Person provides Medicaid-covered community care, home modifications, rent assistance, and transition support for individuals moving from qualified institutions to community living.

Eligibility

• Care recipient must have a qualifying disability
• Applicant age: at least 18

How to apply

• Talk to your transition team to enroll; eligible individuals transition through the CHOICES, ECF CHOICES, or another waiver HCBS program
