Three Medicaid 1915(c) waivers (Comprehensive Aggregate Cap, Self-Determination, Statewide) providing home and community based services to people with intellectual disabilities as an alternative to ICF/IID institutional care

Eligibility

  • Care recipient must have a qualifying disability
  • Care recipient age: under 6

How to apply

  • Meet ICF/IID level of care and financial eligibility with a TennCare-approved Pre-Admission Evaluation (PAE), then contact your TennCare MCO (BlueCare 888-747-8955; United Healthcare Community Plan 800-690-1606; Wellpoint 833-731-2153) to enroll; questions to the LTSS Help Desk 1-877-224-0219
