Structured Family Caregiver (SFC) Program

A Medicaid-funded benefit that pays a daily stipend to an unpaid family caregiver of a Medicaid HCBS waiver participant, including care coordination and oversight.

Daily stipend to caregiver (approx. $40-$70 per day); care coordination and health support; monthly home-visit oversight.

Eligibility

• Care recipient must be enrolled in a Medicaid HCBS waiver
• Caregiver must be a family member living with the care recipient
• Care recipient must meet nursing-facility level of care
