Personal Care Agency Services

Personal Care Agency Services provide assistance with activities of daily living for eligible individuals through home or community-based locations.

How to apply

• The recipient must participate in a needs assessment conducted by the Department of Human Services, Division of Long Term Services and Supports (LTSS), at least once every twelve months. Referrals should be made through Dakota at Home, which can also be contacted at 1-833-663-9673 to determine if additional services may be authorized once the 500-hour limit is reached.
