Money Follows the Person

Money Follows the Person supports adults 18 or older to transition from institutional care to community-based services and supports.

  • Applicant age: at least 18

There are transition teams to assist people who are directly served through the Rhode Island Medicaid Office and those who are served by a managed care organization.
