Veteran Directed Care

Veteran Directed Care provides veterans who would otherwise need nursing home level of care with a flexible budget for supportive skilled and non-skilled services, managed by the veteran or caregiver with case management support.

Eligibility

• Care recipient must have a qualifying disability

How to apply

• Assisted through the process by a case manager from the local Area Agency on Aging; currently accessed through a consult placed by a VA Primary Care Provider
