Independent Choices

Independent Choices allows eligible Medicaid beneficiaries under 65 with disabilities or seniors living in their own home to direct and manage their own services and supports through a self-directed budget.

Eligibility

• Care recipient must have a qualifying disability
• Care recipient age: under 65
• Relationship: spouse, parent, other relative

How to apply

• Case manager conducts an in-person CA/PS Title XIX functional needs assessment in the individual's home or care setting to determine eligibility
