1915(i) Home and Community-Based Services

1915(i) Home and Community-Based Services provides in-home personal care, community transportation, home-delivered meals, housing support, transition services, and pest eradication to adults ages 21 and over with serious and persistent mental illness with income not exceeding 150% of the Federal Poverty Line.

Eligibility

  • Household income at or below 150% of the Federal Poverty Level
  • Care recipient must have a qualifying disability
  • Care recipient age: at least 21

  • The IQA receives requests for HCBS 1915(i) service authorization eligibility determinations for individuals who are potentially eligible for 1915(i) HCBS services from a referrer
