1915(c) Aged and Physically Disabled Waiver

CMS-approved Medicaid 1915(c) waiver run by APD for aged and physically disabled adults. It funds waiver case management, one-time community transition set-up expenses for people leaving institutional or provider-operated settings for in-home care, and housing support services.

Eligibility

• Care recipient must have a qualifying disability
• Applicant must have a qualifying disability
• Applicant age: at least 18
• Care recipient age: at least 18

• Apply or update benefits at benefits.oregon.gov or call 800-699-9075 (or contact the ADRC at 855-673-2372). A Department or Area Agency on Aging case manager assesses service needs with the CA/PS assessment, determines eligibility, and offers service choices; the assessment calculates the service priority level
