Money Follows the Person¶
Money Follows the Person supports SoonerCare members aged 19 and older with disabilities in transitioning from institutional settings to community living, including frail elderly individuals 65 and older, individuals 19-64 with physical disabilities, and individuals 19 and older with intellectual disabilities.
Eligibility¶
- Care recipient must have a qualifying disability
- Applicant age: at least 19
- Care recipient age: at least 19
- Relationship: other relative
How to apply¶
- A family member, health care professional, or any other interested party may make a referral; apply online via the Living Choice common intake form or the online referral form
