Structured Family Caregiving

Structured Family Caregiving is a Medicaid service for individuals at least 18 years old who need nursing facility level of care, allowing spouses or relatives with legal decision-making authority to provide care in the individual's home if they complete caregiver training.

Eligibility

  • Care recipient must have a qualifying disability
  • Must live with care recipient
  • Relationship: other relative

How to apply

  • Once found eligible for PASSPORT, a case manager develops a package of in-home services from local service providers, which can include Structured Family Caregiving
