Self-Directed Services

Self-Directed Services allows individuals of all ages with disabilities to maintain independence at home by choosing and directing their own trained aides to assist with activities of daily living and other support, with up to $10,000 in annual funds available.

Eligibility

• Care recipient must have a qualifying disability
• Relationship: spouse, parent, other relative, non relative

How to apply

• Enrolled HCBS waiver individuals work with ODM's designated Financial Management Services (FMS) agency and their waiver case manager to set up self-direction
