Program for the All-Inclusive Care for the Elderly (PACE)

The Program for the All-Inclusive Care for the Elderly (PACE) is administered by the New York State Department of Health and serves adults aged 55 years or older who reside within the Plan's service area. Contact the Department of Health to learn about eligibility and available services.

  • Care recipient age: at least 55

How to apply

  • Individuals seeking to enroll in a PACE plan may contact the New York Independent Assessor (NYIA) for an assessment, or contact a PACE plan directly, to confirm Nursing Facility Level of Care and the need for more than 120 days of community-based long-term services and supports.
