New York State Money Follows the Person (MFP) Program

The New York State Money Follows the Person (MFP) Program helps eligible New York residents aged 18 or older transition from nursing homes, hospitals, and intermediate care facilities to homes in the community, such as houses or apartments. MFP funds Medicaid home- and community-based services for people with physical or developmental disabilities, traumatic brain injury, or older adults.

Eligibility

  • Care recipient must have a qualifying disability
  • Applicant age: at least 18

How to apply

  • Anyone can make a referral to the Open Doors Transition Center at any time, by phone, email, fax, or online
