  • Individuals seeking Community Based Long-Term Care are a non-MAGI Medicaid eligibility group who apply through their Local Department of Social Services (LDSS) (in NYC, the Human Resources Administration), not through the NY State of Health marketplace, using the Medicaid Application for Non-MAGI Eligibility Group (DOH-4220).
