Personal Care Services Self-Directed Model

The Personal Care Services Self-Directed Model provides assistance and support for persons with disabilities living independently in their homes. Ongoing services are subject to annual reassessment and require a functional assessment by a Nevada Medicaid-certified physical or occupational therapist.

• Care recipient must have a qualifying disability

How to apply

• Requires a functional assessment by a Nevada Medicaid-certified physical or occupational therapist; provider submits a prior authorization request (Form FA-24) via the Provider Web Portal
