Section 1115 demonstration including Managed Long Term Services and Supports (MLTSS)

Eligibility

• Care recipient must have a qualifying disability
• Care recipient age: at least 21

How to apply

• Individuals 21 and older: contact your local County Area Agency on Aging (AAA) - Aging and Disability Resource Connection (ADRC) to be clinically screened for MLTSS. Children birth through 20: contact the Division of Disabilities Services (DDS) at 1-888-285-3036 to speak with an Information and Referral Specialist.
