Money Follows the Person

The Money Follows the Person (MFP) demonstration supports state strategies to rebalance LTSS systems from institutional to community-based care.

How to apply

• MFP has no self-service individual application; New Hampshire contracts with its seven Aging and Disability Resource Centers (ADRCs) to perform Money Follows the Person Hospital and Nursing Home Outreach and Education, identifying and connecting institutionalized individuals who could transition to community-based care (via the Choices for Independence waiver) with MFP and its housing navigation services.
