How to apply

• Complete the Application for Nebraska Medicaid for Aged and Disabled (which lists Program for All Inclusive Care for Elderly (PACE) as an option to apply for) and deliver it to the local DHHS office, mail it to DHHS Medicaid Eligibility Program, PO Box 2992, Omaha, NE 68103-2992, or fax it to (402) 742-2351; call (855) 632-7633 for help. Medicaid generally can also be applied for online at iServe, by phone, or at a local DHHS office.
