Medicaid State Plan Personal Care Services

Medicaid State Plan Personal Care Services provides Medicaid-eligible North Dakotans with disabilities in-home personal care assistance from family members or other caregivers, authorized up to 120-300 hours per month depending on care level.

• Care recipient must have a qualifying disability

• Application for services shall be made to the department utilizing "Application for Services," SFN 1047.
