1915(i) State plan Home and Community-Based Services

North Dakota's 1915(i) provides behavioral health services and supports for qualifying people with certain behavioral health conditions, coordinated by an assigned care coordinator.

Eligibility

  • Household income at or below 150% of the Federal Poverty Level
  • Care recipient must have a qualifying disability
  • Care recipient age: at least 0
  • Relationship: parent, child

How to apply

  • All members get a 1915(i) care coordinator who helps write the person-centered plan of care; you can find a care coordinator provider in your county, or contact the 1915(i) navigator by emailing [email protected] or calling (701) 328-7068 or (800) 755-2604
