Community First Choice Services and Personal Care Services

Community First Choice Services and Personal Care Services provide Medicaid-eligible individuals with assistance for activities of daily living through personal care services in home and community-based settings.

Eligibility

• Care recipient must have a qualifying disability
• Relationship: spouse, parent, child

How to apply

• If you are Medicaid enrolled and have a health condition that causes limitations in activities, call Mountain Pacific at 1-800-219-7035 for an assessment. If you do not yet have Medicaid, visit apply.mt.gov to apply or call the Public Assistance Helpline at 1-888-706-1535.
