Big Sky Waiver

1915(c) waiver that lets people who would otherwise be institutionalized live in their own home and community, with services coordinated by Case Management Teams (nurse and social worker)

Eligibility

• Care recipient must have a qualifying disability

How to apply

• 1. Make a referral to BSW by contacting Mountain Pacific at (800) 497-8322 or (406) 443-4020 (level of care screening). 2. County Office of Public Assistance eligibility specialists determine Medicaid eligibility. 3. Case Management Team assesses in person, places applicant on wait list (SLTC-146 criteria tool) when slots are full, then holds an intake meeting to develop the service plan.
