Bridge to Independence

Bridge to Independence provides enhanced services that facilitate the transition of qualified beneficiaries from institutional care to a home and community-based setting, with a 365-day follow-up after transition.

How to apply

• The Mississippi Access to Care (MAC) Center is the designated Local Contact Agency that receives and distributes Long Term Care Transition to Community Referrals (TCR) to the appropriate case management agencies (CMA); the fillable referral form and instructions are posted by MDHS.
