Money Follows the Person

Money Follows the Person helps eligible Missouri Medicaid recipients transition from nursing facilities and institutions to home and community-based settings.

Eligibility

• Care recipient must have a qualifying disability
• Care recipient age: at least 63

How to apply

• Contact a team member in your regional office to meet, discuss options, and be assessed for eligibility; assessment conducted within 10 business days of referral
