Minnesota health care program offering flexible options for people to direct their own home care; replaced Personal Care Assistance (PCA) and the Consumer Support Grant (CSG) as of Oct 1, 2024.

Eligibility

• Care recipient must have a qualifying disability
• Relationship: spouse, parent

How to apply

• The county/Tribal Nation assessor conducts an assessment to determine eligibility; the person then selects CFSS services if eligible and chooses a consultation services provider
