Self-Directed Services

Self-Directed Services enables Michigan Medicaid recipients to hire and direct their own home and community-based care providers.

Eligibility

• Applicant age: at least 18
• Care recipient age: under 18
• Relationship: spouse, parent

How to apply

• Self-direction is elected during the person-centered planning (PCP) process with the Community Mental Health Services Program (CMHSP); the individual chooses a Financial Management Services (FMS) provider and develops an Individual Plan of Service (IPOS) and individual budget
