Kentucky Transitions

Kentucky Transitions helps elderly, physically disabled, and individuals with mental illness or acquired brain injury move from nursing facilities or institutions into their own homes with up to $2,500 in transition assistance, based on need.

Eligibility

• Care recipient must have a qualifying disability

How to apply

• If you are interested in moving out of a nursing facility or institution, email Kentucky Transitions or call (502) 564-0330 or toll-free (877) 564-0330. If you know someone who could benefit from Kentucky Transitions, you can complete a referral form and return it via email to [email protected] or by mail to: Kentucky Transitions 275 East Main Street, 6W-B Frankfort, Kentucky 40621.
