Program of All-Inclusive Care for the Elderly (PACE)

The Program of All-Inclusive Care for the Elderly (PACE) s designed for seniors with chronic care needs and their families to allow qualified individuals the opportunity to remain in the community for as long as possible. The program promotes quality, comprehensive health services for older adults.

Eligibility

  • Applicant age: at least 55
  • Care recipient age: at least 55

How to apply

  • To apply for the PACE program, contact your local PACE Organization. Via Christi HOPE serves Sedgwick County (316-858-1111); Midland Care Connection serves Douglas, Jackson, Jefferson, Leavenworth, Lyon, Marshall, Nemaha, Osage, Shawnee, Pottawatomie, Wabaunsee and Wyandotte counties (785-232-2044); Bluestem PACE serves McPherson, Ottawa, Saline, Rice, Marion, Reno and Harvey counties (844-588-7223).
