Program of All-Inclusive Care for the Elderly (PACE)

The Program of All-Inclusive Care for the Elderly (PACE®) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.

Eligibility

  • Applicant age: at least 55
  • Care recipient age: at least 55

How to apply

  • There is no single statewide enrollment portal; PACE is delivered by local PACE organizations, each serving specific counties or zip codes. To enroll, contact the PACE organization serving your county directly (see the Indiana PACE Programs directory of addresses and phone numbers by service area on the DDARS PACE page). Applicants must be certified to need nursing facility level of care and able to live safely in the community at the time of enrollment.
