Money Follows the Person

Money Follows the Person assists Indiana Medicaid eligible individuals transitioning from nursing facilities or developmental disabilities institutions to live in the community, helping locate housing and arranging funding for medical, rehabilitation, home health and other services for 365 days.

Eligibility

• Care recipient must have a qualifying disability

How to apply

• A transition nurse and transition specialist work with the applicant while still in the institution to identify needs and supports before discharge
