1915(i) State Plan Home and Community-Based Services

Indiana's 1915(i) program is the Child Mental Health Wraparound (CMHW) services benefit, operated by the Division of Mental Health and Addiction (DMHA), for youth with significant mental health needs.

Eligibility

  • Household income at or below 150% of the Federal Poverty Level
  • Care recipient must have a qualifying disability
  • Relationship: parent

How to apply

  • All referrals for CMHW services must go through the DMHA-authorized state-wide access site, which conducts a face-to-face evaluation including the CANS assessment and the CMHW application
