Family Caregiver Home Health Aide Services¶
Program providing reimbursement for legally responsible family caregivers credentialed as Certified Nurse Aides to provide home health aide services
Eligibility¶
- Household income at or below 150% of the Federal Poverty Level
- Care recipient must have a qualifying disability
- Care recipient age: under 21
How to apply¶
- Referrals may come from hospitals, physicians, schools, nursing agencies, and the OA website. An assessor at the OA performs the person-centered comprehensive assessment that is sent to the MA, along with supporting documentation, including medical reports. The OA submits the completed eligibility enrollment packet to the MA for review.
