Money Follows the Person

Money Follows the Person provides assistance locating eligible housing and with the move, including transition costs, for eligible individuals over age 60 or with physical disabilities, serious mental illness, or intellectual/developmental disabilities who are Medicaid-eligible and reside in nursing facilities for at least 90 days.

• Care recipient must have a qualifying disability
