1915(i) State plan Home and Community-Based Services

Illinois 1915(i) State Plan HCBS benefit reimbursing legally responsible family caregivers as a Certified Nursing Assistant or Certified Nurse Aide to provide personal care or home health aide services to a medically fragile relative under 21 who receives in-home shift nursing coordinated by UIC/DSCC.

Eligibility

• Household income at or below 150% of the Federal Poverty Level
• Care recipient must have a qualifying disability
• Care recipient age: under 21
• Relationship: parent

How to apply

• Referral-based: referrals may come from hospitals, physicians, schools, nursing agencies, or the OA (UIC/DSCC) website; an OA assessor performs a person-centered comprehensive assessment sent to the state Medicaid agency for review and final eligibility determination
