1915(i) State Plan HCBS Benefit provides respite services, community-based supports, and family education to families caring for children with developmental disabilities and functional and behavioral needs.

Eligibility

• Care recipient must have a qualifying disability
• Relationship: child, other relative

How to apply

• Step 1: Apply for Medicaid. Step 2: Apply for the Children's with Disabilities Program. Step 3: Complete an interview with a independent assessor provider. To apply for help with the Children's Developmental Disabilities Program, download and fill out an application. Return the application by email, mail, hand delivery, or fax.
