PACE HAWAII PROGRAM

PACE HAWAII PROGRAM provides multidisciplinary care to eligible adults aged 55 and older who need nursing facility-level care, including primary medical care, adult day health services, therapy services, home care, and non-emergency transportation through the Department of Human Services.

Eligibility

• Care recipient must have a qualifying disability

How to apply

• An eligible individual shall be enrolled into PACE effective the month in which the application to the PACE provider is received
