Family Home Health Aide Program

Program allowing family members or guardians to be trained and employed as home health aides to provide services for medically fragile children

Eligibility

• Care recipient must have a qualifying disability
• Applicant age: at least 18
• Care recipient age: under 21
• Relationship: other relative, non relative

How to apply

• Confirm PDN eligibility for your child, contact a participating home health agency, complete FHHA training and background screening, submit the FHHA SSN Request Form to DCF, then begin services as scheduled by the agency
