Personal Care Services

Personal Care Services provides Medicaid coverage of up to 24 hours of personal care services per day for Florida Medicaid recipients under 21 with a medical condition or disability limiting their ability to perform activities of daily living, delivered in home or community settings with a physician's order.

Eligibility

• Care recipient must have a qualifying disability
• Applicant age: under 21
• Relationship: other relative, non relative

How to apply

• Personal Care Services is one of the minimum covered services under every Florida Medicaid Managed Medical Assistance (MMA) and Long-Term Care (LTC) plan. There is no separate consumer application: the recipient must already be enrolled in Florida Medicaid (an MMA or LTC plan), be under the care of a physician, and have a physician's order for personal care services; the plan/provider then completes the required assessment and Plan of Care.
