Consumer-Directed Care Plus

Consumer-Directed Care Plus is a Medicaid program offering self-directed personal assistance services that allows recipients to hire and pay neighbors, friends, spouses, or relatives as providers to help them remain in their own home.

Eligibility

  • Care recipient must have a qualifying disability
  • Relationship: spouse, child, other relative, non relative

How to apply

  • Contact the CDC+ Helpline at 1-866-761-7043 for information and resources on getting started on CDC+. First, enroll in Florida's Agency for Persons with Disabilities (APD) Developmental Disabilities/Home and Community-Based Services iBudget Waiver; consumers must be fully enrolled with APD (not in pre-enrollment) before application to CDC+ may begin. Then select a CDC+ Representative if needed, take initial training, choose a CDC+ Consultant, and complete the Application and Enrollment packet. Submit the completed Application and Enrollment packet forms and Direct Deposit Request form to the CDC+ Consultant, who submits them to the State Office on the Consumer's behalf.
