1915(i) State plan Home and Community-Based Services

The 1915(i) State plan Home and Community-Based Services provides employment support services including employment navigation, financial coaching, and benefits counseling to individuals age 14 and above with visual impairment, physical disability, or intellectual/developmental disability whose income does not exceed 150 percent of the Federal Poverty Line.

Eligibility

• Household income at or below 150% of the Federal Poverty Level
• Care recipient must have a qualifying disability
• Care recipient age: at least 14
