Program of All-Inclusive Care for the Elderly (PACE)¶
The Program of All-Inclusive Care for the Elderly is a nationally recognized model of care that integrates Medicare and Medicaid benefits for eligible beneficiaries with a broader array of benefits than typically available under either program, with care managed by a comprehensive, inter-disciplinary team of clinical professionals.
Eligibility¶
- Care recipient must have a qualifying disability
- Applicant age: at least 55
