Connecticut Home Care Program for Elders

Connecticut Home Care Program for Elders provides home and community-based services including homemaker, personal care, companion services, meals on wheels, adult day care, mental health counseling, and care management to eligible seniors.

Eligibility

• Care recipient must have a qualifying disability
• Care recipient age: at least 65

How to apply

• Apply faster online at connect.ct.gov, or by mailing the W-1LTSS Application for Long-Term Services and Supports to the Greater Hartford Community Options Applications Unit (for community homecare) or the appropriate DSS office.
